2024 HRSA RHND Care Navigation Grant FAQ

Community Listening Sessions:

In 2024 Wilderness Health held five community listening sessions to explore barriers to accessing mental and behavioral health care. Involving the voices of our communities and patients in the development of new care practices is integral to advancing rural health. Throughout this grant programming we aim to intentionally include as much of our community as possible.

Wilderness Health has prepared a report that reviews the strategies of change identified during the sessions. To ensure that we captured the voice of the communities we visited we are asking for feedback. The survey is 16 questions and is open until [Date] to gather feedback to inform our future grant programming and activities.
