Perinatal Substance Use Project Coordinator
This is a grant funded position with funding available through May 2028. The Perinatal Substance Use Project Coordinator (PC) will work to improve outcomes around substance use and maternal health via the Minnesota Department of Health “Enhancing Outcomes for Pregnant/Postpartum Families Impacted by Substance Use Disorder” (EOPI-SUD) grant. The Project Coordinator will work under the Executive Director of WH and with the input of the EOPISUD project workgroup and stakeholders. The PC functions as a core member of Wilderness Health’s grant team. The PC will regularly interface and collaborate with health care leaders, physicians, staff, counties and community health and substance use care organizations. They will act to coordinate and lead the workplan and outcomes goals of the EOPI-SUD project. They will coordinate the project to measure, assess, and reduce negative outcomes for pregnant/postpartum families with substance use disorder. The PC will connect with our health system facilities and county public health agencies in Carlton, Itasca, Lake, St. Louis, and other Northeastern MN counties. This includes assessing situations at each participating healthcare facility, coordinating creation and distribution of surveys and education materials, and work with stakeholders to deliver evidence-based education and harm reduction materials across the region. The PC will serve as a trusted resource to staff, providers, and the community. The PC will collaborate with the WH Program Associate and RN Care Navigator to coordinate and develop grant activities and materials. Work in this position will be a hybrid of in-person and virtual. Travel is estimated to be 20% of work hours.
Major Job Duties & Responsibilities:
1. Interface and collaborate with stakeholders.
- Connect to health system staff, leaders, and physicians at participating member hospitals.
- Connect with county health departments, social service agencies, and substance use care providers.
- Build trust between health workers, county agencies, and the community.
- Organize meetings that bring partners together for updates, work group and committee gatherings.
- Facilitate the project workgroup to establish evidence-based approaches to SUD care and harm reduction in prenatal/postpartum populations.
- Provide guidance to the workgroups and streamline startup structures and procedures.
2. Assess current situation, attitudes, and gaps towards SUDs in health facilities.
- Evaluate and suggest changes to health facility SUD workflows.
- Stay current with policies being developed to support and/or limit the ability of providers to treat pregnant/postpartum families impacted by SUDs.
- Conduct a needs assessment for the use of a community health worker.
3. Education and improving attitudes around SUDs.
- Coordinate the development, distribution and data collation of a pre and post SUD attitude surveys to health workers and applicable stakeholders.
- Coordinate best practices development alongside identified experts.
- Coordinate the production of educational and training resources for users including providers, staff and patients.
- Facilitate the dispersion of education and harm reduction materials to health system staff and families impacted by SUDs.
- Coordinate and attend community events to provide community awareness and harm reduction materials supporting pregnant and postpartum families impacted by SUDs.
- Serve as a trusted resource to community members, participating health systems, and counties.
4. Management and Coordination.
- Coordinate the development strategy of the EOPI-SUD grant including facilitation and tracking of grant reporting, keeping the project workplan on track, and ensuring distribution of education materials and harm reduction supplies.
- Construct an implementation plan that aligns with clinical and organizational needs and goals.
- Demonstrate financial responsibility and accountability through the effective and efficient use of resources in daily procedures and practices.
- Communicate with grantor about project progress and coordinate completion of required progress and financial reports.
5. Other duties as may be assigned.
